马来西亚耳鼻喉科头颈外科协会前主席、耳鼻喉专科医生叶育耀医生(Dr Yap Yoke Yew)指出,鼻咽癌(NPC, Nasopharyngeal Carcinoma)是一种“难以捉摸”的癌症,因为它生长在鼻咽部,一个鼻腔后、喉咙上的隐蔽区域,“既看不到也摸不到”,使早期的检测极具有挑战。

癌级症兆

第一期(早期)患者会出现一些症状如失去听觉、耳鸣、唾液带血、鼻涕带血

第三期(早期)患者的癌细胞扩散到淋巴或进入头脑

第四期(末期)患者的癌细胞进一步扩散到肺、骨、肝

“第一期至第三期,首三年无复发生存率(disease free survival)高达90至95%。如果是5至10年的无复发生存率,则可能会是80至85%。

“在鼻咽癌,即便第三期癌细胞已扩散,全面医治后首3年无复发生存率同样是90至95%。“

我国5大癌症

根据国家癌症登记局(Malaysian National Cancer Registry)近期的报告指出,在2007至2011年,国内有10万3507人确诊患癌症,男女分别是45.2%与54.8%;男女患癌的风险分别是1/10与1/9。

报告显示,鼻咽癌是第5大癌症。5大常见癌症是:

叶育耀指出,根据砂拉越一项调查报告,比达友的鼻咽癌发病率是26人/10万人,比世界上1人/10万人还高。西马是5.4人/10万人,砂拉越是13.5/10万人。

此癌在中国广东省、香港、台湾和东南亚较为普遍,显示与遗传有关。

鼻咽癌的肇因有很多,包括基因、腌制食品如咸鱼、含亚硝胺(Carcinogenic Nitrosamines)蔬菜,以及爱泼斯坦巴尔病毒(Epstein-Barr viral)感染。

在马来西亚,2008年鼻咽癌数据(Nasopharyngeal Carcinoma Database)显示,木业工人是高危群,原因可能与木业所使用的杀虫剂和甲醛(Formaldehyde)有关。

我们还能看到李宗伟叱咤球场吗?

相信国人都想知道,也殷切期盼可以再次看到李宗伟继续为国争光。对于这个问题,叶育耀表示:“可能一开始没有那么健壮,不过他绝对可以继续如以往般,假以时日,再战球场。”

谈到一些过往病例,他指出,他有一病人是健身教练,也有一位医生朋友十年前多患鼻咽癌,他们至今都康复得很好,继续从事原有的职业。

知多一些:

EBV-CA这是一种EB病毒检查,阴性是正常的意思。说明没有那个病毒,是健康的意思。

Epstein-Barr病毒(EBV)为疱疹病毒科嗜淋巴细胞病毒属的成员,为95%以上的成人所携带。它是传染性单核细胞增多症的病原体,还与鼻咽癌、儿童淋巴瘤的发生有密切关系,被列为可能致癌的人类肿瘤病毒之一。

EB病毒不是肿瘤组织产生的,但在鼻咽癌的患者中,90%以上EB病毒检查阳性.因此, VCA/IgA(壳抗原抗体), EA/IgA(早期抗原抗体),EBNA/IgA(核抗原抗体)测定可作为体检筛查鼻咽癌的方法之一。正常值为:VCA/IGA(壳抗原抗体IgA)≤1:10,EA/IGA(早期抗原抗体 IgA)阴性。

KUALA LUMPUR: A type of nose cancer known as Nasopharyngeal Carcinoma (NPC) is most common among Malaysians of Chinese ancestry, said Dr Rekha Balachandran, Ipoh Pantai Hospital Ear, Nose and Throat consultant; and head and neck surgeon.

There is also a high incidence of NPC among the Bidayuh in Sarawak.

“Heredity, lifestyle and Epstein-Barr virus (EBV) infections are factors which cause the cancer. Having a close family member with NPC increases the risk of the disease, and frequent consumption of preserved food such as salted vegetables and dried fish also contributes to it,” she said.

NPC often appears in the nasopharynx, the upper part of the throat behind the nose and near the base of the skull.

Malaysians were stunned by recent reports that national number one badminton star Datuk Lee Chong Wei was diagnosed with early stage nose cancer, with many wishing him a speedy recovery.

The Badminton Association of Malaysia (BAM) president Datuk Seri Mohamad Norza Zakaria said that Lee is currently in Taiwan seeking treatment.

NPC symptoms

The first symptom of NPC is often a painless swelling or lump in the upper neck. Those who have such a lump for more than three weeks are advised to seek medical advice immediately.

“The second symptom is bleeding from the nose. Nasal blockage or stuffiness and reduced hearing, especially on one side only, are symptoms too,” said Dr Rekha.

However, she said NPC can be treated if detected at an early stage.

“The five-year survival rate is actually quite high. For the earliest stage of NPC, stage one and two, the survival rate is 80 to 85 per cent. I have personally seen some patients with NPC survive more than five years… 10 to 20 years. As long it is detected before the tumour spreads too much,” she added.

Difficult to detect

Dr Rekha said NPC is difficult to diagnose early because the initial symptoms do not cause pain or discomfort.

“Patients with the symptoms may not visit a doctor because they think it is not anything dangerous, and also because the tumours grow in places we can’t see. They are very easy to miss,” she added.

Dr Rekha said that a healthy lifestyle would help reduce the risk of contracting NPC.

“Reducing or eliminating some types of foods may lower the NPC risk. Your diet should be balanced. Eat more fresh greens and fruits and avoid preserved foods,” she added.

It is my great honor to have been elected to be the President of the Malaysian Society of Otorhinolaryngologists – Head and Neck Surgeons (MSO-HNS) for the term 2017 – 2018. I am blessed to have worked together with a wonderful Exco, which had been excellent throughout the term.

The Society has grown not only in numbers but it has matured with all the positive vibes from our junior and senior members. Our members come from multiple backgrounds, diverse in race culture, and religion but they are united under the ORL-HNS umbrella.

We have continued the tradition of giving back to the community by organising health camps in rural areas that we deem are in need of healthcare services. This year we went to Pulau Teluk Renjuna, Tumpat, Kelantan with not only the ORL-HNS team from the Society but also we have invited teams for Ophthalmology, Family Medicine, and Dental/OMF.

Providing updates to the general practitioners (GP) around the country has been a recurrent event for the past few years. The reason it continues is due to the overwhelmingly positive feedback from GP and also medical officers from the Kementerian Kesihatan Malaysia who have appreciate the knowledge that they have gained from the seminars.

To encourage the young upcoming ORL-HNS specialists to present their work in the field, the Society also organised the Annual Scientific Meeting on 1st April 2018 at the Royale Chulan Hotel, Kuala Lumpur which has become an anticipated event for the specialists in training. Through the MSO-HNS Research Grant, we also encourage our members to apply for our research grants to help them in their studies. The researchers will apply their evidence-based knowledge for the benefit of the community and this will make our Society as a proactive group that contributes to the nation.

This term we also took the initiative to revamp our website and hope that members can benefit and use it as a platform for us to exchange knowledge, views and to keep in touch among the members.

To cap it off, the finale for this term we will be proudly organising the 10th Malaysian International ORL-HNS Congress and the 38th Annual General Meeting of the MSO-HNS in Hotel Perdana, Kota Bharu, Kelantan on the 3rd to 5th May 2018. In conjunction with the Congress, a pre-congress airway workshop will be held on 1st and 2nd May 2018 at Universiti Sains Malaysia Health Campus Kubang Kerian, Kota Bharu, Kelantan and a post-congress ENT Update for Nurses and Paramedics on 5th May 2018 in Hotel Perdana, Kota Bharu, Kelantan.

Like all great societies, participation from the members is the key to success. Whether it is a constructive criticism or a well deserved appraisal, the 2017 – 2018 Exco takes it all positively and try our best to make this Society a success and lifting it up high as a benchmark for others to follow. I humbly hope that we can move forward in unity, facing all the challenges collectively and become a truly outstanding Society.
